Burnett Dairy Cooperative near Grantsburg, WI.  Nothing special to look at, but the have the best cheddar!

Why go to a store for your cheese? There's nothing better than fresh Wisconsin cheese curds made right before your very eyes at the Union Star Cheese Factory in Zittau, Wisconsin. You haven't had cheese until you've had a fresh sqeaky cheese curd! YUM!!!!!!!!!!!!!!!

One of my favorites is Eastman's Cheese House in Esperance, NY  on US Route 20 in the middle of dairy country. Although  it  is not as big as the ones cited above, it does harken one to an earlier time.

The Cheese Shop in Carmel should be added to this list. They have knowledgeable staff who give customers priority service and they offer up a huge variety of cheese and wine that is local and foreign. Dependent on the event, the staff will do presentations or lectures and the store accepts online orders. Many people would consider their trip to the Monterey Peninsula incomplete if they didn't visit the Cheese Shop in Carmel.

this is by far the best cheese shop in the country.  You can't find a better one in the states and i have been to over half the shops on this list.  Only place better is in France.
